1.1 Annex A – Practice privacy notice
Nelson Medical Group has a legal duty to explain how we use any personal information we collect about you, as a registered patient, at the practice. Staff at this practice maintain records about your health and the treatment you receive in electronic and paper format.
What information do we collect about you?
We will collect information such as personal details, contact details, next of kin, records of appointments, visits, telephone calls, your health records, treatment and medications, test results, X-rays, etc. and any other relevant information to enable us to deliver effective medical care.
How we will use your information
Your data is collected for the purpose of providing direct patient care however, we can disclose this information if it is required by law or is justified in the public interest. The practice may be requested to support research; however, we will always gain your consent before sharing your information with medical research.
Processing your information in this way and obtaining your consent ensures that we comply with Articles 6(1)(c), 6(1)(e) and 9(2)(h) of the GDPR.
All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice. The average pay for GPs working in Nelson Medical Group in the financial year ended 31 March 2017 was £68,782 before tax and NI. This calculation is for 2 par-time GPs.
Over 75 Health Check
Are you over 75?
As of April 2014 all patients over 75 are required to have a named doctor who will have overall responsibility for the care and support that Nelson Medical Group provides them. Most patients are already aware of their named doctor, if you are unsure please ask a member of staff. If your named doctor is not the doctor you usually prefer to see, this is not a problem, you may continue to see the doctor of your choice.
All patients over 75 who do not already attend the surgery for an annual review are welcome to book a health check with the health care assistant. Our health care assistant will check your blood pressure, weight, height, urine and enquire about any health needs. Please book with the receptionist if you wish to have a health check.
Shared Decision Making
Making decisions about your care with your doctor or nurse (shared decision making)
When you visit your doctor’s surgery you will often find that there are decisions to be made about your health and the treatments that might be available to you. This includes when you are choosing between different types of treatment or different ways of managing any condition(s) you have. When these decisions are made it is important that you are part of that process, so that you are able to come to the best decisions based on what is important to you.
Your doctor/nurse is an expert about health and health care. You are an expert in knowing about yourself, the impact that any conditions have on you, and what is important to you in treating your condition and in your wider life. When you and your doctor/nurse work together to share what you both know, and then use all of that information to come to a decision together, this is called ‘Shared Decision Making’.
How to get involved
In order for you to be involved in decisions about your care there are three key things you need to know;
- What are my options?
- What are the possible risks, benefits and consequences of each option?
- How can we make a decision together that is right for me?
With shared decision making your doctor/nurse is there to support you by providing good quality information, helping you understand this information, and giving you support and guidance as you think about what is most important to you. This will help you to understand what choices are available to you, the pros and cons of each option, and then use that information to come to a decision together about the best option for you.

Patient Decision Aids
Patient Decision Aids (PDAs) are designed to help you decide which treatments and care options are best for you.
PDAs are useful because they allow you to pick out the things that are most important to you (your values) and make comparisons about how different treatments might affect these values. Patient decision aids have been developed for a number of common health care decisions and your doctor/nurse may use one or refer you on to one when you talk with them, or you might find it useful to look at one by yourself. If you are looking for more general information about your health or health conditions then the NHS Choices website contains good quality information.
New Evening and Weekend Appointments we now have additional appointments available at a time to suit you – after 6.30pm on weekdays and also on Saturdays and Sundays through the day. Bank holidays are also covered by this new initiative. This has been organised by your own GP practice, in partnership with other practices in North Tyneside. These appointments are all pre-bookable so, if you need an appointment in the evening or the weekend because you are busy during the week, then please continue to arrange it through your own GP practice. Many of these appointments will available on the same day that you call your practice if you need to make an appointment to be seen quickly. We have appointments until 8pm every weekday and on Saturdays and Sundays, to see a doctor (GP), nurse or Health Care Assistant. Some of these appointments are at hubs or central locations to suit you, but they are all provided by local doctors and nurses and they have full access to your patient notes. Please remember to ask your practice administration team. If you are calling to arrange an appointment at the weekend, please call the team at Battle Hill Health Centre on 0191 295 8520 and they will be happy to help.
Did you know that Nelson Medical Group has a patient forum that meets 3 to 4 times a year. This gives both the patients and our staff the opportunity to learn from each other about what is happening at the surgery and with the larger CCG patient forum groups. If you would be interested in joining our group please ask at Reception and they will be able to provide you with further information.
We want to get better at communicating with our patients. We want to make sure you can read and understand the information we send you. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know. We want to know if you need information in braille, large print or easy read. We want to know if you need a British Sign Language interpreter or advocate. We want to know if we can support you to lipread or use a hearing aid or communication tool. Online Services Records Access
Patient information leaflet ‘It’s your choice’
If you wish to, you can now use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and look at your coded medical record online. You can also still use the telephone or call in to the surgery for any of these services as well. It’s your choice.
Being able to see your patient record online, might help you to manage your medical conditions. It also means that you can even access it from anywhere in the world should you require medical treatment on holiday. If you decide not to join or wish to withdraw, this is your choice and practice staff will continue to treat you in the same way as before. This decision will not affect the quality of your care.
You will be given login details, which is unique to you. This will ensure that only you are able to access your record – unless you choose to share your details with a family member or carer. Administration will take 21 days from the date requested.
The practice has the right to remove online access to services. This is rarely necessary but may be the best option if you do not use them responsibly or if there is evidence that access may be harmful to you. This may occur if someone else is forcing you to give them access to your record or if the record may contain something that may be upsetting or harmful to you. The practice will explain the reason for withdrawing access to you and will re-instate access as quickly as possible.
It will be your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately.
If you can’t do this for some reason, we recommend that you contact the practice so that they can remove online access until you are able to reset your password.
If you print out any information from your record, it is also your responsibility to keep this secure. If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all.
The information that you can see online may be misleading if you rely on it alone to complete insurance, employment or legal reports or forms.
Be careful that nobody can see your records on screen when you are using Patient Online and be especially careful if you use a public computer to shut down the browser and switch off the computer after you have finished.

Before you apply for online access to your record, there are some other things to consider.
Although the chances of any of these things happening are very small, you will be asked that you have read and understood the following before you are given login details.
Things to consider
There may be something you have forgotten about in your record that you might find upsetting.
Abnormal results or bad news
If your GP has given you access to test results or letters, you may see something that you find upsetting. This may occur before you have spoken to your doctor or while the surgery is closed and you cannot contact them. If this happens please contact your surgery as soon as possible. The practice may set your record so that certain details are not displayed online. For example, they may do this with test results that you might find worrying until they have had an opportunity to discuss the information with you.
Choosing to share your information with someone
It’s up to you whether or not you share your information with others – perhaps family members or carers. It’s your choice, but also your responsibility to keep the information safe and secure. If it would be helpful to you, you can ask the practice to provide another set of login details to your Online services for another person to act on your behalf. They would be able to book appointments or order repeat prescriptions. They may be able to see your record to help with your healthcare if you wish. Tell your practice what access you would like them to have.
If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.
Your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care. Some of the information within your medical record may be highly technical, written by specialists and not easily understood. If you require further clarification, please contact the surgery for a clearer explanation.
Information about someone else
If you spot something in the record that is not about you or notice any other errors, please log out of the system immediately and contact the practice as soon as possible.

The average pay for GPs working in Nelson Medical Group in the last financial year was £51,553 before tax and National Insurance. This is for 5 part time GPs who worked in the practice for more than six months.
We have been providing a text reminder to patients for appointments for some time. We are now extending this service to provide patients with feedback about their test results and to communicate with our patients. If you change your number please ensure that you notify us.
All patients are allocated a named GP. If you wish to know which GP is designated as your named GP please contact the Reception Team.
We Would Like Your Help
With the ever changing demands on General Practice we feel it increasingly important to involve and work with our patients and community. Could you spare some time to join our Patient Participation Group? If so please contact the Practice and let us know you would be interested in further information on our group meetings. If however this is a little too much for you to commit to but you would still like to join a 'virtual' reference group which will receive emails asking opinion on different subjects affecting the Practice then please leave your email address with one of our Receptionists and they will ensure this is passed on to be added to the group distribution.
Januarys Friends & Family Feedback
Feedback this month has been fantastic and many thanks to those of you who have taken the time to fill out one of our questionnaires. We received 51 questionnaires. 28 responded that they would be extremely likely to recommend our Practice, 18 that they were likely to, 3 fedback that they were neither likely nor unlikely and 1 feedback each stated that they were unlikely and exrremely unlikely to recommend us. Negative feedback for us to consider was that we have no late appointment, however we do have appointments up until the Centre closes at 6pm. Following feedback with regards to patients being cut off by the telephone system we are also looking at whether the telephone system can be changed to prevent this happening in future. All positive feedback has been shared with our team and we would like ot say a great big thanks for your support and positive comments. The team really do appreciate your comments.
Decembers Friends & Family Feedback
Feedback from patients has so far been slow. Only 2 repsonses were received in December. Both were positive in that they would recommend the Surgery to their friends and family, however no furthter comments were provided. Please help us to improve the Surgery and complete a questionnaire next time you use the services. Information provided will be used by both the Practice and our Patient Participation Group to guide us to improvements.
Friends and Family Test
We would like your feedback on the care or treatment we give you any time you visit us or have contact with the practice. It doesn’t take long.
Put us to the test and tell us what is working and what we can improve. You can say what you think without giving your name and we will use the information to plan improvements to our services. The short questionnaire can be collected from either Reception or during a consultation and returned confidentially to the box on Reception.
Ambulance Booking for Routine and Same Day Appointments
Please note that as of the 8th December 2014 all patient transport requests have to be made by the patient, family member or carer directly with the Patient Transport Services (PTS) on 0191 650 0560.
Book Your Appointments Online
We have a new booking and prescription webiste. Please ask a receptionist to register you to access this service. Once registered the receptionist will give you information to create your user name and password. This service is available 24 hours a day 365 days a year. Please remember to allow 2 WORKING DAYS FOR ALL PRESCRIPTIONS.
Please remember that an appointment is for 10 minutes. If you have several complaints that you need to see a clinician about then please ensure that you book an appropriate number of appointments.
You can also find out information about medical conditions and diseases through our patient information leaflets as well as keep up to date with all the latest practice news and developments. You can also allow the nurses to collect travel information by using the Travel Form.
